  2. First Name

  3. Names of parents

  4. Date of birth and Place of Birth (including the province, city and country)

  5. Nationality

  6. Family permanent Address (include telephone number)

  7. Employer (if working)

  8. Duty

  9. Work address

  10. Spouse’s Name before you got married (include place of birth and his/her parents’ name)

  11. Purpose of travel to Poland

  12. Waver if invited by a family member in Poland

  13. Address and other details of your family in Poland who are inviting you.

  14. Means of Transportation to Poland

  15. Passport

  16. If application is waved, when is the date it was waved?

  17. The date and length of stay in Poland as indicated in the waver.

  18. Reasons if denied of a visa to Poland before. When and Where?

  19. Propose of length of stay in Poland (specify the date)

  20. Passport number

  21. Acknowledgement that all the information you gave is true and correct, affix your signature and date.

Note:

  1. All questions in the application form must be answered honestly and clearly.

  2. Date of birth and passport must be unanimous

  3. Birthplace must indicate the municipality, city, province and country.

  4. The names of the parents must be written in full

  5. Be specific with your job title. Avoid vague terms such as employee or staff.

  6. Home address must also include telephone number and area code.

  7. Remember not to write anything on the space for the Department of Consular Officer only.
